Assistance with Ticket Form

Hi, using the form created with Afrin Shoukath at Kaseya. I made a change to remove some fields from the form so users cannot them, but we have a different form for IT where we can see them if needed. Almost everything works except the Notes tab is missing when creating a Service Desk ticket in VSA. Can you please guide me to what is missing? I can provide copies of before and after my changes.
